Supporting Health, Empowering Lives

HCHF's Health Related Programs and Support Groups bring together community members living with chronic illness, navigating caregiving, or working toward better wellness. Our groups are peer-led, professionally guided, and completely free to attend.

From Alzheimer's and Parkinson's to COPD, cardiac wellness, stroke recovery, and cancer support — we offer a safe, welcoming space where participants share experiences, learn from one another, and find strength in community.

All programs are funded entirely through community donations and run by dedicated volunteers and staff who believe that connection is medicine.
